Through the Health Risk Assessment, users fill out a detailed questionnaire about their lifestyle, demographics, and personal medical history and receive a comprehensive and confidential wellness report that includes individualized feedback plus suggestions for specific ways to lower their risk. Administrators can use the aggregate data they get from HIPAA-compliant HRA results to make informed decisions about their wellness programming.

Stand-alone Health Risk Assessment(HRA)
You can get a comprehensive report on your population’s health through the eWellness Portal HRA. The Health Risk Assessment comes with the eWellness portal but is also available as a standalone product. To schedule the Health Risk Assessment program, you can use eWellness Portal’s online scheduling tool where users/employees can subscribe to the event and administrators can manage the process easily.
How it works
Health Risk Assessment will come with a questionnaire, where users/employees fill out detailed information about their lifestyle, demographics, and medical history to calculate their risk for chronic diseases such as diabetes, heart disease or stroke and other health conditions. Based on the answers, the HRA provides feedback and corrective measures to lower health risks.
Administrators/users can use the HRA results to make decisions about what wellness programs to implement such as, healthy eating and weight loss, get fit in 15, walking and running and many more offered through the eWellness Portal.
